Munford Ave., Stockton, CA <br /> Fox Loomis, Inc. (hereafter referred to as FLI) is pleased to provide submittal information for well destruction for the project noted <br /> above.This proposal is based on the information noted below and addresses the work we will perform. <br /> WORK PLAN FOR PROPOSED WELL DESTRUCTION <br /> WELL DESTRUCTION PER EMAIL PROVIDED BY FRANK GIRARDI DATED 3/6/2025 <br /> 1. FLI will have (2 ea.) grout pumps on site. <br /> 2. FLI will set 1" flush-thread pipe,full depth, in each well. <br /> 3. 25 grain det-cord will be set, full depth, in each well. <br /> 4. FLI will tremie-pour neat cement in each well. <br /> 5. Det-cord will be placed in the following holes and set off: <br /> o PZ-1(a) shallow and deep <br /> o PZ-1(b) shallow and deep <br /> o AS-2, AS-3, and AS-4 <br /> o VW-3 <br /> o MW-8, MW-12 <br /> 6. FLI will place plastic sheeting and straw wattles around the following holes to contain water displacement: <br /> o PZ-1(a) shallow and deep <br /> o PZ-1(b) shallow and deep <br /> o AS-2, AS-3, and AS-4 <br /> o VW-3 <br /> o MW-8, MW-12 <br /> 7. FLI will reinstall the tremie pipe and re-grout each well to within 3 ft of the surface with 25 psi of pressure for a <br /> duration of 5 minutes. <br /> 8. If tremie pipes encounter hazardous materials, tremie pipes will be washed by hand with a bleach solution while <br /> employees wear standard PPE. <br /> 9. FLI will remove the top 3' of casing after grout has set and place a mushroom cap. <br /> 10. Any hazardous materials will be containerized and hauled off by a third part. <br /> 11.
