Site Control <br />Decontamination <br />Sanitation <br />Safety Briefings <br />Employee Health and Safety Engagement <br />Hazard Observations (via H&S App or TIP) required at the following frequency on this project: <br />Close Call reporting (via H&S app) goals for this project: <br />Other (specify): <br />Safety Equipment and Supplies <br />The CPM or APM is responsible for reviewing and establishing H&S engagement goals for the project. <br />These goals are summarized below. <br />One TIP per week <br />One near miss per event <br />Arcadis will lead all safety briefings on this project and will document the safety briefing on a Tailgate Safety <br />Briefing form or logbook. Safety briefings will be conducted once at the beginning of each work day unless <br />the Site Safety Officer deems more frequent safety briefings will be required based on work being <br />conducted. All project workers, including Arcadis subcontractors, will be required to attend the safety <br />briefing. Site visitors and project workers not on duty during the morning safety briefing will receive the <br />safety briefing upon their arrival onto the project site for the day. <br />The safety briefing will also address COVID-19 prevention protocols to be used on site as described in <br />current Arcadis recommendations and guidance. <br />Restroom facilities and potable water will be provided by an Arcadis contractor for this project. Unless <br />alternate requirements are stipulated in a plan supplement (i.e. Heat Injury and Illness Prevention Plan), <br />permit or JSA, temporary restroom facilities will be provided with one toilet for every 20 project workers and <br />bottled or non-plumbed potable water will be provided to project workers at 1 gallon/worker/day. <br />Site control requirements are addressed in the applicable task JSA for this project. JSAs requiring site <br />control are attached to this HASP. <br />Decontamination protocols are addressed in the applicable task JSA(s) for this project. The applicable JSAs <br />are attached to this HASP. <br />defines a CMV as any single vehicle with a gross vehicle weight rating (GVW R) ≥10,001 pounds or a truck <br />and trailer combination with a combined GVW R ≥10,001 pounds (GVW R of truck + GVW R of trailer = <br />≥10,001 pounds). <br />21
