Immediately schedule these tests and provide 48 hours notification to the EHD. <br /> This is (Class 2)Violation. <br /> 45 CA Code of Regulations 2636(f)(2)2030025-LLD on DW press piping monitors hrly, detects a 3.0 gph leak, and <br /> restricts/shuts off flow(TCR 9d). <br /> OBSERVATION:The 91 VMI line leak detector failed to detect a leak when tested. <br /> REGULATION GUIDANCE:All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. <br /> CORRECTIVE ACTION: The service technician adjusted the leak detector, retested it, and it passed. <br /> This is a repeat violation (Class 2) Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> OBSERVATION:The designated operator failed to document all the alarms from the attached alarm history on the <br /> August, June, and July designated operator inspection report and failed to check that they were responded to <br /> appropriately.
