SANsJOAQUIN Environmental Health Department <br /> COUNTY <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FRIENDZ FOREVER LOGISTICS INC 1419 S MAIN ST, MANTECA June 24, 2025 <br /> 57 CA Code of Regulations 2715(c) 2030012 - Initial and annual training provided to employees and at least one trained <br /> employee is present <br /> OBSERVATION: The designated operator employee training was not performed for the 2023 and 2022 year. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual <br /> performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a <br /> site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to <br /> emergencies, leak detection, monitoring alarms, spills and overfills. <br /> At least one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, <br /> and submit a copy of the training records to the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d) <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The following <br /> records were not observed onsite: <br /> - Monitoring System Certification test results for 2022 and 2023 <br /> - Secondary Containment test results conducted on 10/23/24 <br /> - Overfill Prevention Equipment test results not observed for 2025 <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br /> (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings <br /> of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings <br /> and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br /> three years on site. <br /> Submit copies of any missing work orders and alarm records to the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> 67 CA Code of Regulations 2712(i) 2030078 - Owner/Operator retained a paper or electronic copy of the permit to operate <br /> at the facility <br /> OBSERVATION: A copy of the UST operating permit was not found on site. <br /> REGULATION GUIDANCE: A copy of the UST operating permit and all attachments and conditions shall be retained at <br /> the facility. <br /> CORRECTIVE ACTION: Provided facility with copy of permit. <br /> This is a Repeat(Class 2)Violation. <br /> EHD-CUPA Inspection Report IS02 Page 6 of 7 FA0003685 PR0231433 4519734 6/24/2025 <br /> 1868 E.
