I've attached a copy of our UST Permit Application Guidance which provides detailed information on our <br /> permit process, including fees. <br /> Payment can be made on our website at Online Payments - LexisNexis Payment Solutions (choose `Service <br /> Request or Well Permit' option) or you can mail us a check.
