Report is emailed to mark.leal@davita.com <br />2. Tracking and treatment records were reviewed from February 2023 – Present. Stericycle is the medical waste hauler. <br />3. The bioaccumulation area had multiple bins that were open but were completely empty. Bins that were empty were not <br />lined with a biohazard bag. <br />4. Only one pharmaceutical bin for facility. The pharmaceutical bin was observed open and was closed by the operator <br />while on site. Noted to the operator that all bins with medical waste should be closed when not being actively used. <br />5. Biohazard bags were ASTM D1709 and ASTMD1922 certified. <br />
