No food, drink, tobacco product, or personal effects are permitted in the procedure area. The practitioner shall not <br />eat, drink, or smoke while performing a procedure. If a client requests to eat, drink, or smoke, the procedure shall be <br />stopped and the procedure site shall be protected from possible contamination while the client leaves the procedure <br />area to eat, drink, or smoke. HSC 119309(i) <br /> <br />OBSERVATIONS: <br />1. Observed a dish with mints and some chocolates that were on the drawer within the procedure area. – CORRECTED <br /> <br />11. Procedure areas in the body art facilty shall be equipped with a sink that is supplied with hot and cold water, <br />containerized liquid soap, and single-use paper towels from a wall mounted touchless dispenser that is accessible to <br />the practitioner. HSC 119314(b)(4) <br /> <br />OBSERVATIONS: <br />1. Paper towels at the procedure sink were not in a wall mounted touchless dispenser. <br /> <br />CORRECTIVE ACTIONS: <br />1. Install a wall mounted paper towel dispenser that is touchless. <br /> <br />18. Prior to the performance of body art, the client shall read, complete and sign an informed consent form . HSC <br />119303(a) <br /> <br />OBSERVATIONS: <br />1. Practitioner did not provide any client consent forms for review. <br /> <br />CORRECTIVE ACTIONS: <br />1. Maintain client consent forms on site to allow inspector to review during inspection. <br /> <br />33. A body art facility shall have counter surfaces and treys that have a smooth, durable, and non absorbent finish. <br />HSC 119314(b)(6) <br /> <br />OBSERVATIONS: <br />1.
