Training certification forms from October 2022 and August 2024 were observed on site but the facility was not able <br /> to provide a training certification form for training conducted in 2023. Supervisor Hasina Nasir stated that the facility's <br /> designated operator usually provides the training but probably did not do so in 2023. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual <br /> performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a <br /> site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to <br /> emergencies, leak detection, monitoring alarms, spills and overfills. <br /> At least one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, <br /> and submit a copy of the training records to the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> 77 CA Code of Regulations 2712(f) 2030073- Facility has corrected cited violations within 30 days of receiving report <br /> OBSERVATION: An inspection was last done on 8/13/2024 and an inspection report was issued identifying information <br /> to be submitted to bring this site into compliance. This information was required to be submitted by 9/12/24. This <br /> information was received on 9/15/24, 3 days late. <br /> REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to implement the <br /> corrections specified in the inspection report and comply with permit conditions. The corrective action shall include all of <br /> the recommendations made in the report.
