Other requirements <br /> may also apply. These requirements cannot be modified. <br /> 1) The water provision declaration form and SB1263 condition will still be needed for this <br /> extension application. <br /> 2) All dairy in-vessel digestion operations shall comply with the Enforcement Agency <br /> Notification requirements set forth in Title 14, California Code of Regulations 17896, <br /> Division 7, Chapter 5.0, Article 3.0 (commencing with section 18100). <br /> 3) Applicant shall contact Natalia Subbotnikova, Program Coordinator, Small Public Water <br /> System Program, at (209) 468-0338, to determine if the existing well can be permitted as a <br /> public water system prior to issuance of building permits. If a public water system is <br /> required, applicant shall submit a Small Public Water System preliminary technical report <br /> to the California State Water Resources Control Board, Division of Drinking Water (Water <br /> Board) at least six months before initiating construction of any water related improvement, <br /> as defined. The issuance of a permit to operate a small public water system by the local <br /> primacy agency (EHD) is prohibited without the concurrence of the Water Board. Please <br /> contact Gena Farley with the SWRCB Division of Drinking Water at <br /> Gena.Farley@waterboards.ca.gov or 209-948-7488, concerning the requirements for <br /> preliminary technical report submittal prior to issuance of building permits. <br /> If the Water Board determines that an onsite well shall be used as the potable water <br /> source, a permit application to operate Small Public Water System shall be submitted to <br /> the EHD for approval prior to issuance of building permits. To issue a permit to operate, <br /> concurrence from the Water Board is required. A yearly permit to operate a public water <br /> system will be required by the EHD prior to sign off of the certificate of final occupancy <br /> (San Joaquin County Development Title, Section 9-602.010 and 9-601.030.). <br /> The supplier must possess adequate financial, managerial, and technical capability to <br /> assure delivery of pure, wholesome, and potable drinking water in accordance with San <br /> Joaquin County Development Title, Sections 9-602.010 and 9-601.030 and C.C.R., Title <br /> 22, and Health and Safety Code, Section 116525 116570. <br /> 1868 E.
