Tracy Blvd., Tracy, CA 95304 <br />PROGRAM COORDINATORS <br />Jeff Carruesco, REHS, RDI <br />Willy Ng, REHS <br />Steven Shih, REHS <br />Elena Manzo, REHS <br />Natalia Subbotnikova, REHS <br />SR0087879 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of the application <br />for a permit to install a new underground storage tank (UST) system at the above noted address, which will <br />consist of: 24,000 gallon 87 -octane (split tank base, with E85) <br />6,000 gallon E85 (split tank end, with 87) <br />15,000 gallon B20 (split tank end, with 91) <br />15,000 gallon 91 -octane (split tank base, with B20) <br />The permit has been approved with the following conditions: <br />1. Contact the local fire district, the San Joaquin Valley Air Pollution Control District, and the local community <br />development department for applicable permits. <br />2. Any changes from the approved UST installation packet shall be submitted to the EHD for written <br />approval prior to commencing work, including changes in contractor, equipment, system layout, <br />etc. All new submittals supersede any previous ones. <br />3. Any individual installing underground storage systems or components must possess, or work under the <br />direct and personal supervision of an individual physically present at the work site at all times while work <br />is being performed who possesses, all of the following: <br />• One of several permissible valid, current contractor licenses issued by the Contractors State <br />License Board (CSLB) <br />• A current UST Installation/Retrofitting certificate issued by the International Code Council (ICC) <br />• A valid, current certificate of training from the manufacturer(s) of the component(s) being installed <br />4. Any individual performing the work of a service technician must possess, or work under the direct and <br />personal supervision of an individual physically present at the work site at all times while work is being <br />performed who possesses, all of the following: <br />• One of several permissible valid, current contractor licenses issued by the CSLB <br />• A current California UST Service Technician certificate issued by the ICC <br />• A valid, current certificate of training from the manufacturer(s) of the component(s) being tested, or <br />the developer of the testing equipment or test method(s) being used <br />5. Individuals working under the direct supervision of a qualified UST installer or UST service technician must, <br />at a minimum, be employed by the person with the CSLB license and possess all manufacturer certificates <br />of training for the equipment on which they are working. <br />1868 E.
