The following is an itemized list of CP22 - AST Non -Qualified violations that have not <br />been addressed for DAMERON HOSPITAL as of July 07, 2025. <br />Open violations from June 19, 2025 inspection <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />j� Supporting documents included }, / <br />Describe actions taken or will be taken to correct violation: 9011 O t -, `m5 G1 e r oli �C t <br />-I � � CAM tkM l ll p1 I I Cln& � r li <br />OBSERVATION: It appears that the oil -handling personnel were not adequately trained, as required: <br />- At the time of inspection, the 10,000 -gallon diesel tank fill port box contained spilled diesel. The facility failed to <br />provide training to the oil -handling personnel regarding the following: 1. The operation and maintenance of equipment to <br />prevent discharges. 2. Discharge procedure protocols. 3. Applicable pollution control laws, rules, and regulations. 4. <br />General facility operations. 5. The contents of the SPCC Plan. <br />REGULATION GUIDANCE: (0(1) At a minimum, train your oil -handling personnel in the operation and maintenance of <br />equipment to prevent discharges; discharge procedure protocols; applicable pollution control laws, rules, and <br />regulations; general facility operations; and, the contents of the facility SPCC Plan. <br />CORRECTIVE ACTION: Immediately provide adequate training to all oil -handling personnel and submit a copy of the <br />training log and training content to the EHD. <br />❑ This violation was corrected JXThis violation will be corrected by (date): <br />❑ Supporting documents included ��y ��� 6� <br />ewDescribe actions taken or will be taken to correct violation: on(e (e ' � e new iman Is <br />OBSERVATION: The facility failed to comply with the following requirements: <br />1. Keep written procedures and records of inspections and integrity tests for at least three years. The facility failed to <br />maintain records of inspections and tests in accordance with written procedures that the certifying engineer developed <br />for the facility, including integrity tests, signed by the appropriate supervisor or inspector. The inspection forms used <br />onsite are not the forms included in the Spill Prevention, Control, and Countermeasure (SPCC) Plan, [and in some <br />instances, these forms do not contain the information required by the SPCC plan]. <br />2. Keep comparison records for bulk storage containers subject to 40 CFR 112.8(c)(6). <br />The facility's "Daily Bulk Diesel Aboveground Tank Inspection Checklist" logs, as discussed in the SPCC Plan, were <br />missing at the time of inspection. <br />REGULATION GUIDANCE: 112.7(e) Conduct inspections and tests required by this part in accordance with written <br />procedures that you or the certifying engineer develop for the facility. <br />CORRECTIVE ACTION: Immediately begin conducting all inspections and tests in accordance with the procedures <br />outlined in the SPCC Plan and begin using forms provided by the SPCC plan. Keep written inspection procedures and <br />records of inspections and testing signed by the appropriate supervisor/inspector for at least three years.
