8.4.3 Program Review <br /> The medical program is reviewed annually to ensure its effectiveness. The Corporate Health and Safety <br /> Manager in coordination with the Human Resources Director is responsible for this review. At minimum, this <br /> review consists of: <br /> • Review of accident and injury records and medical records to determine whether the causes of <br /> accidents and illness were promptly investigated and whether corrective measures were taken <br /> wherever possible; <br /> • Evaluation of the appropriateness of required medical tests based on Site exposures; and <br /> • Review of emergency treatment procedures and emergency contacts list to ensure they were <br /> Site-specific, effective, and current. <br /> 8.5 Personnel Protection <br /> Site safety and health hazards are eliminated or reduced to the greatest extent possible through engineering <br /> controls and work practices. Where hazards are still present, a combination of engineering controls, work <br /> practices and PPE are used to protect employees. Appropriate PPE shall be worn by Site personnel when <br /> there is a potential exposure to chemical, biological or physical hazards (e.g., falling objects, flying particles, <br /> sharp edges, electricity and noise), as determined by the SHSO and the JSAs. The level of personal <br /> protection, type and kind of equipment selected will depend on the hazardous conditions and in some cases <br /> cost, availability, compatibility with other equipment, and performance. An accurate assessment of all these <br /> factors will be made before work can be safely executed. <br /> Roux maintains a comprehensive written PPE program that addresses proper PPE selection, use, <br /> maintenance, storage,fit and inspection. Roux's PPE program can be found within Appendix E. PPE to be <br /> used at the Site will meet the appropriate American National Standards Institute (ANSI) standards and the <br /> following Cal/OSHA standards for minimum PPE requirements. <br /> The minimum level of PPE for entry onto the Site is Level D. The following equipment shall be worn: <br /> • Work uniform (long pants, sleeved shirt); <br /> • Hard hat; <br /> • Steel or composite toe work boots (must comply with American Society for Testing and Materials <br /> [ASTM]) F 2412-05, Standard Test Methods for Foot Protection and ASTM F 2413-05, Standard <br /> Specification for Performance Requirements for Foot Protection); <br /> • Safety glasses (must comply with one of the following ANSI/ISEA Z87.1-2010, ANSI Z87.1-2003, <br /> ANSI Z87.1-2003); <br /> • Boot covers (as needed); <br /> • Hearing protection (as needed); <br /> • High visibility clothing (shirt/vest); and <br /> • Hand protection (e.g., minimum cut resistance meeting ANSI 105-2000 Level 2). <br /> Note that jewelry shall be removed or appropriately secured to prevent it from becoming caught in rotating <br /> equipment or unexpectedly snagged on a fixed object. (e.g., wrist watches, bracelets, rings, chains and <br /> necklaces, open earrings).
