8.9.3 Monitoring the Effectiveness of Decontamination Procedures <br /> Visual examination is used to evaluate the effectiveness of decontamination procedures. Visual examination <br /> is used to ensure that procedures are implemented as described and that they appear to control the spread <br /> of contaminants under changing Site conditions. Visual examination is also used to inspect for signs of <br /> residual contamination or for contaminant permeation of PPE. <br /> Personnel who work in contaminated areas of the Site, either the CRZ or the EZ, are trained in the principles <br /> and practices of decontamination described in this section of the HASP and in related SOPs. If Site <br /> procedures are changed as a result of inspection and monitoring, all affected employees are notified of <br /> these changes. <br /> 8.10 Confined Space Entry <br /> No confined space entry will be performed. <br /> The following is a list of the safety requirements for confined space entry at the Site: <br /> • ROUX PERSONNEL ARE NOT AUTHORIZED TO ENTER ANY Cal/OSHA PERMIT OR NON- <br /> PERMIT REQUIRED CONFINED SPACE; <br /> • The scope of work may require personnel to enter any permitted or non-permitted confined space <br /> for this project; and <br /> • Any changes to the field activities that may necessitate confined space entry will be reported to the <br /> PP and OHSM before any confined space entry is made. <br /> Confined space is defined as any space, depression, or enclosure that: <br /> • Has limited opening for entry and egress; <br /> • Is large enough for and employee to enter and perform assigned work; and <br /> • Is not intended for continuous occupancy. <br /> A permit required confined space is one that meets the definition of a confined space and has one or more <br /> of the following characteristics: <br /> • May contain or produce life-threatening atmospheres due to oxygen deficiency the presence of toxic, <br /> flammable, or corrosive contaminants; <br /> • Contains a material that has the potential for engulfment; <br /> • Has an internal configuration that may cause an entrant to be trapped or asphyxiated by inwardly <br /> converging walls or by a floor that slopes downward and tapers to a smaller cross-section; and <br /> • Contains any other serious safety or health hazards. <br /> Although Roux personnel will not perform confined space entry, it is expected that subcontractors performing <br /> cleaning and mitigation, and/or remedial measures activities may be required to enter structures that are <br /> considered to be a permit or non-permit required confined space. Permitting of the confined space as well <br /> as hazard mitigation for entry will be required to be completed by the subcontractor in accordance with <br /> T8 CCR, Subchapter 4 Construction Safety Orders Article 37-Confined Spaces in Construction. <br /> 5175.0001S102/HSP Site-Specific Health and Safety Plan I ROUX 133 <br />
