Check in with Site Manager/Supervisor to ensure <br /> coordination with other Site activities and to discuss any <br /> special hazards. Ensure that short-service employees <br /> (SSE)are identified. <br /> 1 b. Identify potential traffic sources. <br /> 1 b. Wear PPE, including high visibility clothing or reflective <br /> vest,when around vehicles or operating equipment <br /> 1 b. Use a spotter while moving work vehicles; plan to avoid <br /> backing whenever possible. <br /> 1 b. Maintain a minimum exclusion zone when vehicles are in <br /> motion (i.e., greater than the swing/tip radius of the <br /> equipment). When backing up the truck rig with an <br /> attached trailer, use a second spotter if there is tight <br /> clearance simultaneously on multiple sides of the <br /> equipment or if turning angles limit driver-to-spotter <br /> visibility. <br /> 1 b. Delineate work area with traffic cones,flags, caution <br /> tape, and/or other barriers. <br /> 1 b. Position"Work Area"signs at Site entrances, if possible, <br /> or at either side of the work area. <br /> 1 b. Position the largest vehicle to protect against oncoming <br /> traffic. <br /> 1 b. Face traffic, maintain eye contact with oncoming <br /> vehicles, use a spotter, and establish a safe exit route. <br /> ' Each Job or Operation consists of a set of tasks/steps. Be sure to list all the steps needed to perform job. <br /> ' A hazard is a potential danger. Break hazards into six types: Contact-victim is struck by or strikes an object; <br /> Caught-victim is caught on,caught in or caught between objects;Fall-victim falls to ground or lower level(includes slips and trips);Exertion-excessive strain or stress/ergonomics/lifting <br /> techniques;Exposure-inhalation/skin hazards;Energy Source—electricity,pressure,compression/tension. <br /> Using the first two columns as a guide,decide what actions or procedures are necessary to eliminate or minimize the risk. List the recommended safe operating procedures. Say exactly what needs to <br /> be done-such as"use two persons to lift."
