SAN 10AQUIN <br />c_ c_� � � i y - <br />Activity Date <br />Environmental Health Department <br />Complaint Record Daily Time and Activity Summary from 8/22/2013 to <br />8/22/2018 for COPFX2L5M located at <br />350 NORTH UNION STREET, MANTECA <br />Recorded by <br />08/19/2018 Received a page at 11:48pm from (209) 468-6677. Called San Joaquin General PE 2546 BAKER <br />12 min Hospital and they said there was a PG&E spill at 350 N. Union Street in Manteca. SC 978 <br />Colt Hawkins ( from PG&E called it in. Approximately 40 gallons of <br />insulating oil pre -1972, PCB unknown released into a dirt bottom vault <br />transformer. <br />I called Colt Hawkins at 11:51 pm. He confirmed the spill. He said that the CAL <br />OES spill number is 18-5611. Crews are on site now doing the clean-up and have <br />excavated approximately a foot of dirt already. Release did not impact waterways <br />or storm drains. Bo McKeirnan from PG&E is on site ( . <br />Called Bo McKeirnan at 11:55pm. He said that they were done excavating the <br />clean-up and would like me to come out an ok it so they can continue fixing the <br />transformer. <br />At 11:57pm I called Muni to let him know what was going on. <br />08/20/2018 Left my house at 12:06am. PE 2546 BAKER <br />168 min SC 978 <br />Arrived on site and met with Bo McKeirnan of PG&E. Excavation of the impacted <br />soil inside the dirt bottom vault had already been completed by PG&E. Site will be <br />leveled out and then backfilled. A sample of the insulating oil will be sent in for <br />PCB analytical. Spill area covered an approximately area of a three foot diameter <br />circle. No waterways or storm drains were impacted. PG&E to provide San <br />Joaquin County Environmental Health Department (SJC EHD) with PCB <br />analyticals and final disposal records for the waste generated. Three drums of <br />waste was generated and was labeled and transported as hazardous waste. <br />Waste will be taken to PG&E yard at 10901 E. HWY 120 in Manteca. <br />Generated a complaint, started prop 65 & Emergency Response Record. <br />Finished notice to abate and emailed it to Alex Steele <br />Left site at 1:42am. Arrived at home 2:06am. <br />12:00am-2:48am <br />08/21/2018 Disposal records for the three drums of waste will be verified on next routine PE 2546 BAKER <br />24 min inspection for PR0507063. SC 544 <br />Complete site map. Finished up photo log. <br />08/22/2018 Received call from Alex Steele saying that the spill report would be amended PE 2546 BAKER <br />6 min because the amount was only 1 gallon and not 40 gallons. He emailed me the SC 544 <br />analyticals which were non -detect for PCB. <br />Page 1 of 1 <br />
