DO Identification form was not uploaded to CERS. Water was found in the diesel fill, diesel STP, 91 STP, and 87 fill sumps and liquid were observed in the 13/14 <br /> UDC. <br /> ➢ Please see attached work order dated 1/13/2025. <br /> 3. DO reports for January 2022-October 2022 could not be located during the inspection. <br /> ➢ Site manager was able to locate the missing DO reports. Please see attached photos of the reports. <br /> 4. Maintenance and monitoring records for the last three years were not found on site. -Overfill prevention test <br /> results for 11/2/2022 following 91 drop tube replacement(SR0085738) <br /> ➢ Our maintenance contractor could not locate testing records from 11/2/022.
