Provide Subcontractor Oversight <br />During Lift/Load Placement <br />- Falling Loads <br />- Faulty Equipment <br />- Heavy equipment hazards could <br />be present <br />- Inadequate Communication <br />- Lifting equipment (cranes) <br />- Line-of-Fire <br />- Moving parts of crane <br />- Personal Injury <br />- Struck by Falling Load <br />- Unauthorized Entry <br />- Unqualified Operator <br />1. Review operator and rigger's credentials onsite prior to lift. <br />Operator and rigger must have a valid certification. <br />2. Ensure crane operator and qualified rigger has reviewed the <br />lift plan and documented inspections of rigging has been <br />completed prior to lift. Rigging equipment must be rated for <br />load as described in subcontractor lift plan. <br />3. All other heavy equipment operations not associated with lift <br />must cease prior to lift, if activities are within 200 feet of the <br />drop zone. <br />4. Non-essential personnel shall not be permitted within 75 feet <br />of outer perimeter of drop zone established by subcontractor. <br />5. Mental Risk Assessments (MRA's) and Activity Hazard <br />Analysis (AHA's) tool must be utilized to help mitigate risks to <br />prevent personal injury and property damage. Discuss "what <br />could go wrong?" and "what's the worst that could happen if <br />something does go wrong?". <br />6. Ensure nobody stands directly beneath a suspended load, <br />keep lift areas clear of all non-essential personnel. <br />7.Identify the final resting place of load <br />L <br />3.
