Set-up and Secure Work <br />Zone <br />- Slips/Trips/Falls <br />- Vehicle and Equipment Traffic <br />- Pinch Points <br />- Biological Hazards <br />- Miscommunication <br />1. Visually scan the area for hazards, be aware of uneven <br />surfaces, watch where you walk, wear boots with adequate tread <br />and a raised heel. Ensure areas are free of obstructions such as <br />tools, materials and equipment. <br />2. Use cones, delineators, snow fence, caution tape, and signage <br />to establish a work area with adequate space. Be aware of your <br />surroundings at all times. Maintain situational awareness <br />regarding vehicle and equipment traffic, If eye contact cannot <br />be maintained with drivers or operators use a spotter or work <br />vehicle as a physical barrier between yourself and traffic. <br />3. Be aware of hand placement at all times, do not place them <br />between objects that could cause pinching or crushing. <br />4. Inspect the work area upon arrival, be aware of Bio-hazards, <br />do not pick up hypodermic needles, stay clear of flying/stinging <br />insects, do not work in areas where snakes or poisonous plants <br />are present, Do not attempt to remove any biological hazards <br />without involving the Superintendent or Foreman. <br />5. Discuss the scope of work with all parties prior to the task, if <br />using a subcontractor to perform this task, ensure to review the <br />subcontractor AHA as well. If any conflict arises with regard to <br />means or methods the safest approach must be followed. <br />L <br />2. Access UST Opening - Inhalation of Hazardous Vapors/Gases <br />- Fire/Explosion <br />- Inadequate Tools or Equipment <br />- Slips/Trips/Falls <br />1. Open Lid (if applicable) and obtain a field reading using from <br />breathing zone and tank opening using a (4) Gas Meter. <br />Reference field reading(s) with site action levels in HASP and <br />determine if it is safe to continue. If LEL concentrations are at <br />or above 10%, Stop work and seek further guidance from the <br />ICS Safety Officer. <br />2. Always use non-sparking (brass) hand tools to remove <br />hardware, keep a 20lbs. ABC Fire Extinguisher charged and <br />readily available during UST removal activities. Important note: <br />Smoking or open flames are not allowed per ICS Policy. <br />3. Use the right tool for the job, do not use broken or <br />compromised tools. <br />4.
