Inspect area for utilities and <br />clearance for heavy equipment <br />- Contact with underground utilities could cause injury to <br />worker and damage to property or equipment. <br />- Verify that there are no overhead power or <br />communication lines <br />1. Physically verify the location and depth of existing utilities prior to starting <br />excavation through geophysical and utility survey. <br />2. Call National One Call 48 hours prior to job start - dial 811. By law, responses <br />from the utility owner are required before commencing the subsurface activities. <br />All utility companies in the 811 should be contacted prior to commencing <br />subsurface activities to determine if they have utilities in the area if they do not <br />provide a response to the 811 call. Review as-built drawings. Protect all existing <br />utilities during excavation. Perform excavation withing 4 feet of existing utilities by <br />hand and/or non-aggressive methods. Protect all underground utilities as soil is <br />removed <br />L <br />2. Set up exclusion zone(s), Work <br />areas, path of travel <br />- Injury or exposure to public or other onsite personnel <br />- Slips/Trips/Falls <br />- Struck by Equipment <br />- Eye Injuries <br />- Sharp objects <br />- Handling heavy objects <br />1. Set up and delineate exclusion zones using traffic control devices such as <br />barricades, caution tape, snow fence, delineators and signage. <br />2. Clear walkways work areas of equipment, tools, vegetation, excavated material <br />and debris. Mark, identify, or barricade other obstructions. Maintain 3 point <br />contact when ascending/descending ladders/mounting/dismounting from heavy <br />equipment. Halt exterior work in high winds, lightning, severe weather. <br />3. Wear reflective hi-vis vests worn when exposed to vehicular traffic. Isolate <br />equipment swings areas. Make eye contact with operators before approaching <br />equipment. Understand and review hand signals. <br />4.Wear face shield, goggles when operating powered clearing/grubbing <br />equipment. <br />5. Wear cut resistant work gloves when the possibility of lacerations or other injury <br />may be caused by sharp edges or objects. Maintain all hand and power tools in a <br />safe condition. Keep guards in place during use. Close doors, windows on heavy <br />equipment to prevent injuries from tree branches and other vegetation. <br />6.
