Trench/ Excavate - Utilities - underground (power, natural gas, water, etc.) <br />- Contact with underground utilities could cause injury to <br />worker and damage to property or equipment. <br />- Excavation Wall Collapse <br />- Struck by Equipment <br />- Inhalation and contact with Hazardous Substances <br />- Hot weather <br />- Cold weather <br />- Strains from use of tools, such as shovels, could occur <br />- Heavy equipment hazards could be present <br />- Workers could fall into excavation <br />- Workers could be injured if they enter excavations that <br />are not protected as required by Cal-OSHA and EM <br />385-1-1, Section 25 <br />1. Identify all utilities around the site before work commences using historical <br />maps, survey maps, markin outs, etc. Cease work immediately if unknown utility <br />markers are uncovered. Use passive digging techniques such as hand digging or <br />air knifing if working within 2 feet of a known utility. <br />2. Use a designated spotter when excavating difficult to see areas. <br />3. Construct diversion ditches or dikes to prevent surface water from entering <br />excavation. Verify proper drainage of area adjacent to excavation. Collect ground <br />water/rain water from excavation and dispose of properly. Keep excavated soil at <br />least 2 feet away from the edge of the excavation; prevent excessive loading of the <br />excavation face. Provide sufficient stairs, ladders, or ramps when workers enter <br />excavations over 4 feet in depth. Place ladders no more than 25 feet apart <br />laterally. Treat excavations over 4 feet deep as confined spaces. Complete confined <br />space permit entry procedure. Monitor atmosphere for flammable/toxic vapors, <br />and oxygen deficiency. Slope, bench, shore, or sheet excavations over 5 feet deep <br />if worker entry is required. Assign a competent person to inspect, determine <br />proper soil classification, proper sloping, the correct shoring, or sheeting. Inspect <br />excavations (when personnel entry is required) daily, or any time conditions <br />change. Provide at least two means of exit for personnel working in excavations. <br />Should competent person find evidence of a situation that could result in a possible <br />cave-in, indications of failure of protective systems, hazardous atmospheres, or the <br />other hazardous conditions, exposed employees shall be removed from the <br />hazardous area until the necessary precautions have been taken to ensure their <br />safety. <br />4. Wear reflective hi-vis vest when exposed to vehicular traffic, Isolate equipment <br />swing areas, Make eye contact with operators before approaching equipment, <br />Understand, review and use hand signals. <br />5. Provide workers proper skin, eye, and respiratory protection based on the <br />exposure hazards present. Review hazardous properties of site contaminants with <br />workers before operations begin. Use light water spray to moisten soil and prevent <br />fugitive dust emissions. <br />6. Monitor workers for Heat/Cold stress. Provide fluids to prevent worker <br />dehydration. Establish work/rest schedule. <br />7. Maintain steady pace when using tools and take adequate rest periods. If <br />possible, rotate tasks among the workers. Use appropriate tools for the task and <br />maintain tools in good condition. <br />8. Equip all heavy equipment on this project with rollover protection systems and <br />back up alarms. Stay clear of moving equipment, unless necessary. If working near <br />equipment, workers must be in visual contact with the operator. Inspect all <br />equipment daily, before use, to ensure that proper maintenance is being <br />performed. Make eye contact with the operator; heavy equipment has right of way. <br />Workers will not work under any equipment or loads. <br />9. As required by EM 385-1, Fall Protection, if the excavation is 6 feet or greater in <br />depth, workers must be at least 6 feet from the edge of the excavation. If workers <br />must be closer, they must use personal fall protection (full body harness, lanyard <br />or retractable life line, anchorage point). If excavation is less than 6 feet deep, <br />workers should stand at least 2 feet from the edge of the excavation. <br />10. Workers can only enter excavations that are directly supervised by a <br />competent person identified above. The Competent person must verify that the <br />excavation is sloped or that an approved shoring system is installed per <br />specifications.
