HSC 117960 - LQG shall file MWMP with the EA. <br />OBSERVATION: The Medical Waste Management Plan (MWMP) is incomplete. The plan is missing the name of the person <br />responsible for the plan, the business address, type of business, types of waste and the certification statement. The plan is a <br />plan that covers all facilities operated by Lodi Memorial Hospital and is not site specific. <br />CORRECTIVE ACTION: Revise the MWMP to ensure compliance with all required elements in HSC 117960. Submit the <br />revised plan to the Environmental Health Department (EHD). <br /> <br /> <br />Submit evidence of corrections to the EHD by <br />06/13/2025. <br /> <br /> <br />Notes: <br />1. Emailed report to Richard Barker at barkerr01@ah.org <br />2. Inspected Building A/B (Employee Health, Occupational Health) and Building D (wound care). Per Alex Rodriguez <br />(EVS Director), Building C used to be pediatrics but is currently vacant. <br />3. Inspected Employee Health, Occupational Health, and wound care areas and their interim biohazardous waste storage <br />rooms. <br />4. Facility has an oxygen therapy room, however, it was not inspected because they were treating patients at the time of <br />the inspections. <br />5. Also inspected the pharmacy, however, they did not have any biohazardous waste. <br />6. Inspected accumulation storage area located in a gated area outside. <br />7. Did not observe any waste inside all of the biohazardous containers on site at the time of the inspection. Containers <br />were properly labeled, lined, and stored. <br />8. Waste types: biohazardous waste and sharps. <br />9. Facility maintains more than 2 yrs of tracking/disposal records. Reviewed records from 2/13/24 – present. <br />10.
