SA k, J O A Q U I N Environmental Health Department <br /> COUN _1 Y <br /> Complaint Record Daily Time and Activity Summary from 2/14/2018 to <br /> 2/14/2019 for C00048048 located at <br /> 810 E FRENCH CAMP RD <br /> Activity Date Recorded by <br /> 12/18/2018 Received an ER page for PG&E. Called and spoke with Celia Barajas who was PE 2546 ALANIZ <br /> 60 min filling in for Alex Steele(925-519-4750) at 9:30 a.m. A car hit a blue sticker, pole SC 544 <br /> mounted transformer releasing 1.5 gallons of mineral oil. She said that the spill <br /> was reported to her at 8:19 a.m. The transformer was located at 810 E French <br /> Camp Rd in Stockton, CA 95206. She did not know any more specifics about the <br /> spill and told me to call Dave Larsen 209-401-9719. Called him and received no <br /> answer. Texted Dave and he said that he was waiting for the work to finish. Called <br /> Celia at 11:11 a.m. and asked for an update. She said that the transformer was <br /> still being replaced. Left for the site at 12:06 p.m. and arrived at 12:22 p.m. Met <br /> with the foreman, Mike Shafe. He said that Dave had already cleaned up the spill. <br /> No signs of mineral oil were observed on the ground. Called Celia at 12:30 p.m. <br /> and she said that the waste was taken to the 4040 West Ln facility. She did not <br /> know how much waste was generated. She was informed that PG&E will be <br /> getting a notice to abate. Her email is cpm3@pge.com. Texted Dave and he said <br /> that one 55 gallon drum of waste was generated and that he was going to be <br /> sampling it tomorrow. Left site at approximately 12:36 p.m. <br /> 12/19/2018 Worked on writing complaint daily from 12/18/2018. PE 2546 ALANIZ <br /> 18 min Received complaint from Tricia Dai. SC 544 <br /> 12/19/2018 Received complaint that was received 12/18/2018 from PG&E- Celia about a 1.5 PE 2546 DAI <br /> 18 min gallon oil spill from a blue sticker transformer located at 810 E French Camp Rd SC 544 <br /> 95231 near APN# 17749007. 1 gave Celia a call (952) 519-4750 to ask if anyone <br /> has responded to the complaint yet. She let me know that John Alaniz and <br /> another co-worker were able to over see the clean up done by Dave Larsen and <br /> his team for that location. <br /> 12/24/2018 Wrote notice to abate and emailed to it Celia Barajas, <CPM3@pge.com>. PE 2546 ALANIZ <br /> 48 min Confirmed replied that she had received the email. Completed Prop 65. SC 544 <br /> Looked up geocode tracker. <br /> 12/28/2018 Completed photo log. Moved jpgs to hdrive. PE 2546 ALANIZ <br /> 18 min SC 544 <br /> 01/10/2019 Called Celia Barajas asking for the PCB results. She said she would check with PE 2546 ALANIZ <br /> 6 min Alex Steele to see if they had come in yet. SC 544 <br /> Her number is (925)519-4750. <br /> 02/14/2019 Received the PCB test results from Celia Barajas <CPM3@pge.com> on PE 2546 ALANIZ <br /> 54 min 1/10/2019. The results indicate that the mineral oil was ND for PCBs. Printed out SC 544 <br /> all the complaint information and abated the complaint. <br /> Page 1 of 1 <br />
