SAN10 A Q U I N Environmental Health Department <br /> i= 0U 'Ilj 'I ' <br /> Complaint Record Daily Time and Activity Summary from 12/7/2016 to <br /> 12/7/2018 for COBYIRGRX located at <br /> 1000 E Roth Rd <br /> Activity Date Recorded by <br /> 12/04/2018 (2:18 am-2:54 am) PE 2546 MANZO <br /> 36 min SC 907 <br /> 2:15 AM received a page from the SJ General Hospital (SJGH); called back at <br /> 468-6007 and received spill report information. This report came from SJ sheriff's <br /> office, according to SJGH operator, a '/2-gallon diesel spill occurred at 1000 Roth <br /> Road in French Camp. Called and spoke with Debbie with sheriff's department, at <br /> 468-4400, she's reporting that no spill response is being requested by the facility, <br /> and this report is for notification only. According to Debbie, a '/<gallon of distilled <br /> fuel spilled sheriff's department incident number is#34. Sandy Anderson with the <br /> Union Pacific is the reporting party, her phone number is 888-877-7267. 2:23 AM <br /> called Sandy with Union Pacific Call Center she's requesting the RIMS number to <br /> be provided before she can release any information, notified her that I do not have <br /> the number as the result had to spend some time getting vetted in. Called General <br /> Hospital and provided my ER Phone number, so they could connect me when <br /> Sandy with Union Pacific calls. Received a call from Sandy she is reporting that <br /> this sill had occurred at approximately 1:44 am today a %-gallon of diesel fuel <br /> spelled from a fueling truck onto the ground. No exposures were reported, no <br /> waterways were impacted. Absorbent pads were used for cleanup, she doesn't <br /> know who did the cleanup at this time; the RIMS number for this spill is <br /> 2018-12-04-017SLA. Sandy is not able to provide me with any facility contact <br /> information at this time. 2:41 AM called Mike Algots with Union Pacific at <br /> 916-580-9999 left a voice message, letting him know which incident I'm calling <br /> about, also asking to verify who is doing the cleanup and what is being done with <br /> the generated waste, provided my contact information. Called Paul Holt with Union <br /> Pacific , also left a voice message with similar information. 2:45 AM documented <br /> the complaint 2:55 AM stop. <br /> 12/04/2018 8:05 AM received a voice message from Paul Holt (with UP). Paul indicated that PE 2546 MANZO <br /> 18 min the responsible party was Savage Fuel Contractor, they cleaned the spill up while SC 007 <br /> on site. Matthew Chesco, at 209-292-1646, is their point of contact for Savage <br /> Fuel. Discussed my spill response and provided an update to Garrett Backus with <br /> EHD. Since I have an inspection scheduled for today, discussed my spill response <br /> and what still needs to be done with John Alaniz ("C" person)with EHD, John will <br /> issue a notice to abate for site cleanup and waste disposal and provide copies to <br /> the UP and the responsible party. <br /> 12/04/2018 (8:30 a.m.-8:54 a.m.) Received spill report 18-8222 from Elena Manzo. PE 2546 ALANIZ <br /> 120 min Approximately %gallon of diesel was released from a broken gasket. Received SC 544 <br /> complaint information from Elena. Called Matthew Chesko with Savage Services <br /> (209) 292-1646. Spoke with the supervisor Donald Williams. His phone number is <br /> (209) 688-6477 and his email is donaldwilliams@savageservices.com. He said <br /> that the spill was stopped and contained. He was going to call Paul Holt to <br /> discuss what to do with the waste. <br /> Received a phone call from Matthew asking for a call back to discuss the spill. <br /> Received a call from Cameron J. McGovern, Environmental Field Operations <br /> Manager, cjmcgove@up.com (510)406-5350, with Union Pacific (1:45 p.m.). He <br /> told me that two absorbent pads of waste was taken to the 833 E Eight Facility. <br /> The waste was put into an oily debris bin and will be taken out as nonhazardous. I <br /> told him to send me the profile results for the waste bin. He sent them to me via <br /> email. Discussed with Garrett Backus. No follow up would be required by the <br /> EHD. I sent a notice to abate to David Williams. Started to fill out complaint <br /> information. Looked up property information on Parcel Viewer. Reviewed waste <br /> analytics. <br /> Page 1 of 2 <br />
