SANJ O A Q U I N Environmental Health Department <br /> —C(-:t_) NTY <br /> Complaint Record Daily Time and Activity Summary from 6/21/2013 to <br /> 6/21/2020 for COBG42OTM located at <br /> Activity Date Recorded by <br /> 04/03/2018 Initial notification received at approximately 10:30 am. PE 2546 RUVALCABA <br /> 48 min SC 544 <br /> Called Paul Holt, Hazmat manager with Union Pacific Railroad (UPRR) at <br /> 916-540-0490, went straight to voicemail. Called Michael Algots, Hazardous <br /> Materials Management with UPRR at(916) 580-9999. Left a message. <br /> Arrived on site at approximately 2:00 pm. Signed in and was told that the container <br /> was in the leaker area of the facility, asked if the cleanup had been conducted but <br /> could not be provided an answer. Observed two green sealed shipping containers. <br /> There was no evidence of leak from the containers that could be observed. <br /> Drums onsite did not indicate that the waste was placed in them, per labeling. Left <br /> facility at approximately 2:30 pm. <br /> Received call from Micheal Algots at approximately 2:45 pm. Micheal stated that <br /> Facility personnel had conducted the clean up, waste was placed in trash. The <br /> majority of the release was contained in the original packaging. The product was <br /> confirmed to be wine. It was stated that he had not been to the site but was <br /> informed that an area the size of a dollar bill had escaped from the shipping <br /> container. The responsible party was identified and the information would be <br /> relayed to the department. <br /> Discussed with lead senior.
