SANJOAQUIN Environmental Health Department <br /> . <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 7/28/2013 to <br /> 7/28/2020 for C00050416 located at <br /> 1000 E ROTH RD <br /> Activity Date Recorded by <br /> 09/04/2019 2:00 AM- Received a page. Called the SJC General Hospital and received the PE 2546 FLORIDO <br /> 36 min following spill complaint information: Mac McDonough of Union Pacific Rail Road SC 907 <br /> (UPRR) reported a leak of an unknown liquid and quantity from the bottom of a <br /> container that was stacked on top of a second container at 1000 E Roth Rd in <br /> French Camp; no impact to ground or waterways, no injuries, and no fire. <br /> Called and spoke to Mac(888-877-7267)who confirmed the above information, <br /> which was first reported to him by Mathew Roberts (209-546-7206), a UPRR <br /> Supervisor, who was on site. According to Mac, Patriot Environmental was going <br /> to conduct the clean-up beginning between 6-7am, per Michael Algots, the UPRR <br /> Hazmat Specialist(916-580-9999).According to Mac, the two impacted containers <br /> were located in the leak pad area of UPRR. <br /> Called Mathew Roberts, who also confirmed the above spill information. Mathew <br /> stated the unknown liquid appeared to be brown where wet and white where dry. <br /> Sent Michael Algots a text message on the EHD ER (phone, asking him if he <br /> needed a response from EHD immediately, or after 8am. <br /> 2:24 AM- END. <br /> 6:00 -7:30 AM(Only 12 minutes spent total): Received text messages back from <br /> Michael Algots stating Patriot was on site and would still be on site at/after 8AM <br /> conducting the clean-up; the leak source had not been found yet; and the waybill <br /> shows the leaking container contains non-hazardous mixed freight. Michael also <br /> provided photos of the leaking container EMHU261895, as well as a copy of the <br /> waybill. Joe Mcguffin (916-597-5520) of Patriot was overseeing the clean-up on <br /> site. Per Michael, Roadrunner Transportation is the responsible party (RP). <br /> Michael had yet to obtain a contact person from the RP. <br /> 09/04/2019 Informed Tricia Dai, EHS and who has the C pager today, of the spill complaint PE 2546 FLORIDO <br /> 12 min information.
