However, a manifest was not <br /> available for review. <br /> CORRECTIVE ACTIONS: <br /> Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. Documentation of proper <br /> disposal shall be maintained for 3 years. Ensure sharps disposal manifests are kept for 3 years and available for review by the <br /> EHD. <br /> 18. HSC 119303-Client records approved and available-Consent form and questionnaire <br /> OBSERVATIONS: <br /> The lot number(s)for the needle cartridge(s) used for the procedure were not attached or written on the client records. <br /> CORRECTIVE ACTIONS: <br /> The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are available <br /> for inspection. Ensure the needle package with the lot number is attached to the client record or written on the record. <br /> 32. HSC 119314-Floors and walls clean and in good repair, adequate light <br /> OBSERVATIONS: <br /> An area near the procedure room door needs paint. -CORRECTED <br /> CORRECTIVE ACTIONS: <br /> A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br /> 35. HSC 119313-Operation and employee training records present <br /> OBSERVATIONS: <br /> Ms.Williams began utilizing Mediwaste Disposal LLC for sharps waste disposal and began using Opticide for disinfectant at the <br /> facility. This information was not updated in the Infection Prevention and Control Plan. —CORRECTED <br /> CORRECTIVE ACTIONS: <br /> Operation and employee training records shall be maintained for 3 years and made available for inspection. A written Infection <br /> Prevention and Control Plan (IPCP)shall be maintained, followed and updated. <br /> No corrections are required to be submitted at this time. <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
