MEDICAL WASTE SERVICE AGREEMENT The following is an understanding between Sharps Solutions LLC, a California Company and Jane Brows , located at, 37 W Yokuts Ave, Suite #1, Stockton, CA, 95207. The parties acknowledge the following terms pertaining to the provision of medical waste services as identified herein. SUPPLY AND TRANSPORT TERMS The pricing included herein includes tub use, liner, pickup, transport, tracking, treatment, disposal and chain of custody documentation. There are no additional charges for a standard pickup. If an onsite container is overflowing or an extreme weight for that particular container size, an additional full or partial container charge may be applied with prior customer approval. If the scheduled pickup of waste cannot be made (i.e., unavailable, office closed, etc.), a pickup charge of $50.00 will be applied. SERVICE TERMS Prior to pickup, customer shall have all waste ready for transportation according to DHS, Medical Waste Management Act and DOT guidelines, said guidelines will be provided by Sharps Solutions and on our website at www.sharps-solutions.com. Customer acknowledges that if Sharps Solutions personnel are requested to package or re-package waste to meet regulatory requirements, which engages our Service Tech, a $45.00 charge may be applied. This charge will only apply when specific approval has been granted by the customer prior to repackaging. Jane Bows and Sharps Solutions... Hereby agree to conform to all regulatory requirements regarding the proper handling and orderly treatment of all Medical Waste in accordance with the State of California, Department of Health Services, Medical Waste Management Act, and all other regulations governing Medical Waste in the State of California. Sharps Solutions and its operating agents adhere and conform to the latest specifications and regulations of the State of California Department of Health Services, Medical Waste Management Branch in addition to all local Regulatory Ordinances for the operation of permitted Medical Waste Transportation, Treatment and Transfer Station operations. Sharps Solutions operates under the authorization and conditions of the California Department of Public Health, MWMP, Medical Waste Transporter #5997. It shall be the responsibility of the Customer (Generator) to conform to the standards and specifications of all applicable regulations that apply to a Generator of Medical Waste. To assist Generator, Sharps Solutions will provide customer with a copy of the State of California, Department of Health Services, Medical Waste Management Act.
