HSC 119312(g) - Certificates of registration for all practitioners performing body art in that facility shall also be <br />prominently displayed either near the health permit or at the individual practitioner’s procedure area if each practitioner <br />has a designated area. <br /> <br />OBSERVATIONS: <br />1. A blood borne pathogen certificate is was not viewed posted within the facility. <br /> <br />CORRECTIVE ACTIONS: <br />1. Provide a copy of the blood borne pathogen certificate to be posted in facility. Provide proof of correction to EHD. <br /> <br /> <br /> <br />35. HSC 119313(e) – Records of training required pursuant to this section shall be maintained for three years and shall <br />be available for inspection upon request of the enforcement officer. <br /> <br />OBSERVATIONS: <br />1. IPCP (Infection prevention control plan) was not available for view onsite. <br /> <br />CORRECTIVE ACTIONS: <br />1. Please provided an updated completed copy of the IPCP to EHD and have IPCP available for review onsite. Provide <br />proof of correction to EHD. <br /> <br /> <br />Facility currently unpermitted and unable to open until all <br />corrections are submitted to EHD. <br /> <br /> <br />Notes: <br /> <br />1. Facilty was noted as an unpermitted facility and owner of facility was contacted to register through Environmental Health <br />Department. <br />3. All walls in facility are smooth, cleanable and nonabsorbent. <br />4. Adequate restroom and hand wash sink properly supplied. <br />5. Practitioner conducted a set up and tear down. 10 mins for wet contact time of cavicide was given. <br />6. Biohazard bags observed within the lined trash can and the edge of the work table not needed as waste that is not <br />considered biohazardous is being placed in them. Please replace both bags with regular bags. Paper towels used to <br />clean clients can be placed in regular waste bin. <br />7. ID verification was observed on site during inspection. Forms are electronic and were viewed on practitioner cell phone. <br />8. Noted to practitioner that Lot numbers of the needles used on clients need to be recorded for every client. <br /> <br /> <br /> <br />
