Spano, Drinking Water Field Operations Branch <br />Gene Reade, Drinking Water Field Operations Branch <br />Mark Barcellos, R.E.H.S., Unit 1 Supervisor, <br />Provide a new source that will meet the standard for drinking water. <br />Submit a permit amendment application and amendment fee of $258 to <br />EHD, attention Unit IV, along with a name of the licensed well driller who <br />will construct the new source and a date that the well will be constructed. <br />The well shall be constructed under permit and inspection from this <br />Department. <br />Option (b) will only be considered if option (a) is not possible. <br />Treatment of the water to remove nitrates is not an option. <br />Please be aware that approval by this Department is required before enacting any of the <br />above options.
