Lincoln Rd., Stockton, CA 95207 <br />PROGRAM COORDINATORS <br />Jeff Carruesco, REHS, RDI <br />Willy Ng, REHS <br />Steven Shih, REHS <br />Elena Manzo, REHS <br />Natalia Subbotnikova, REHS <br />SR2501401, amended <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br />application for a minor plan check permit to do the following on the underground storage tank (UST) <br />system at the above noted address: <br />• Replace supply/return line test reducer split fitting on diesel emergency generator <br />• Break concrete near supply & return lines to investigate for possible leaks <br />The minor plan check permit has been approved with the following conditions: <br />1. Contact the local fire district, the San Joaquin Valley Air Pollution Control District, and the local <br />community development department for any applicable permits and requirements. <br />1. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />2. Provide written notification to ustna <br />sigov.org for the following repairs and testing (a 48-hour <br />notice is required). Please be aware: an inspector may not be present for testing. <br />a. Secondary <br />containment piping test on supply and return lines <br />This permit does NOT approve the facility to perform any piping repairs. If piping repairs are <br />needed, an addendum to this permit, or a new permit submittal, will be required. <br />3. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses all applicable licenses and <br />certifications required by the Contractors State License Board <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service <br />Technician certificate issued by the International Code Council (ICC) <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced <br />1868 E.
