The following is an itemized list of CP22 - HMBP violations that have not been <br /> addressed for JACK IN THE BOX #4301 as of January 08, 2026. <br /> Open violations from September 30, 2025 inspection <br /> Violation#12-1010010-Electronically submitted adequate response plan/procedures for <br /> release/threatened release of hazmat. <br /> OBSERVATION: The emergency response(ER)procedures were inadequate due to the following: <br /> -The CUPA phone number is not included <br /> -The nearest hospital name and phone number are not listed <br /> -Section E3 is not filled in <br /> REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a release <br /> or threatened release of hazardous materials, including, but not limited to,the following: -immediate notification of <br /> local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a <br /> release or threatened release to minimize any potential harm or damage to persons, property, or the environment - <br /> evacuation plans and procedures, including immediate notice,for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#14-1020002-Provided initial and annual training and maintained training records for a <br /> minimum of three years. <br /> OBSERVATION:According to Hector Medina,training is not conducted. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and <br /> procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. (B)Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br /> notice,for the business site. This training shall be documented electronically or by hard copy and shall be made <br /> available for a minimum of three years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> Page 3 of 4 <br />
