Due to missing the following: - Loading Area, Evacuation Staging Area, Emergency Shut offs (Gas, Water, Electric) REGULATION GUIDANCE: A site map that contains north orientation, adjacent streets, access and exit points, evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map requirements the governing body of the unified program agency finds necessary CORRECTIVE ACTION: Electronically submit a site map with all required content. This is a (Minor) Violation. 16 19 CCR 1 5030.10(b); HSC 6.95 25505(a)(4) 1020002-Provided initial and annual training and maintained training records for a minimum of three years OBSERVATION: The business failed to provide annual training to all employees in safety procedures in the event of a release or threatened release of a hazardous material including familiarity with the emergency response plan - Danny Cuthbert indicated that the aforementioned training is not conducted and record of annual training for the past 3 years could not be provided. REGULATION GUIDANCE: (a) The business plan shall include a training program which is reasonable and appropriate for the size of the business and the nature of the hazardous materials handled. The training program 27 shall take into consideration the responsibilities of the employees to be trained. The training program shall, at a minimum, include: (1) methods for safe handling of hazardous materials; (2) procedures for coordination with local emergency response organizations; (3) use of emergency response equipment and supplies under the control of the handler, and (4) all procedures required by Section 5030.9 of this Article. (b) The business plan shall include provisions for ensuring that appropriate personnel receive initial and refresher training. CORRECTIVE ACTION: Establish and electronically submit an employee training program containing provisions to ensure initial and annual training for all employees in safety procedures in the event of a release or threatened release of a hazardous material and document and maintain training records for a minimum of three years. This is a Repeat (Class 2) Violation. Overall Inspection Comments Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by January 16, 2026.
