No. <br />0518 <br />1515 South River rd <br />West Sacramento, CA 95691 <br />(916) 371-5747 <br />Additional Waste haulers may be contacted in the event the alternate medical waste <br />hauler is unavailable [List of additional waste hauler contacts are located inside <br />Hazardous Waste Manifest Binder managed by Environmental Services (EVS) <br />department]. <br />(d) Do you have tracking documents for all hazardous waste handled at your facility? <br />The Kaiser Compliance document for record keeping states Manifest <br />documentation must be kept for life! Uniformed hazardous waste manifest and <br />land disposal restriction (LDR) forms will be retained permanently. <br />(e) Describe your hazardous waste emergency action plan, including procedures for <br />handling spills, exposures, equipment failures, etc. <br />The Contingency Plan shall be carried out immediately whenever there is a fire, explosion, spill or <br />release of hazardous material/waste or hazardous material/ waste constituents to the air, soil or <br />surface water which could threaten human health or the environment. This plan minimizes the <br />hazards to human health and the environment that may occur as a result of emergencies involving <br />hazardous materials/wastes. The regulations that specify the contingency plan requirements are <br />found in the California Code of Regulations (CCR), Title 22, Chapter 15, Article 4, beginning with <br />§66265.50. <br />As a generator that stores hazardous waste on-site, we are required to complete the contingency <br />plan. We are to keep one copy of the plan and submit a copy of the plan to each agency that may <br />provide emergency response to our facilities. The plan must be revised whenever the plan fails <br />during an emergency, the facility changes, the contents of the plan change or the regulations <br />change. <br />The Emergency Coordinator (Safety Officer) has authority to implement the contingency plan. <br />In the event of an emergency, the following personnel shall take responsibility as Emergency <br />Coordinator (Appendix A and B). In the absence of the Emergency Coordinator, the <br />Administrator-On-Call is responsible. <br />The cell phone number of those assigned as Emergency Coordinators will be maintained on file in <br />the Operator's Directory. Whenever there is an imminent or actual emergency situation, the <br />emergency coordinator (or designee) shall immediately: <br />Activate internal facility alarms or communication systems, where applicable, to notify all <br />facility personnel; and <br />Notify appropriate State and local agencies with designated response roles if their help is needed. <br />Stockton Tommydon MOB Page 2 of 5
