Hazardous Materials & Waste Contingency Plan <br />Kaiser Permanente Stockton Medical Offices <br />1/01/2025-12/31/2025 <br />The administrator (or designee) shall notify the Department of Health Services, and appropriate State <br />and local authorities, which facility is in compliance with the above noted section before operations <br />are resumed in the affected area(s) of the facility. <br />The administrator (or designee) shall document the time, date and details of any incident that requires <br />implementing the contingency plan. Within 15 days after the incident the administrator (or designee) <br />shall submit a written report on the incident to the Department of Health Services. The report shall <br />include: <br />Name, address, and telephone number of the administrator (or designee) <br />Name, address, and telephone number of the facility <br />Date, time, and type of incident (e.g., fire, explosion) <br />Name and quantity of material(s) involved <br />The extent of injuries if any <br />An assessment of actual or potential hazards to human health or the environment, where this is <br />applicable and, estimated quantity and disposition of recovered material that resulted from the <br />incident. <br />Spill / Release: <br />Procedures will outline responsibilities to be assumed by those in the medical facility and will <br />specifically address the role of external agencies contracted to address emergency spills. <br />Spills must be cleaned up immediately by a properly trained person using appropriate personal <br />protective equipment in relation to the spill. <br />Spills are cleaned according to Hazardous Waste Management Policy EC.HZ.056: Spill <br />Management: Hazardous Materials & Waste. <br />In the event of an employee exposure, the employee shall immediately be evaluated at the <br />Employee Health Services or an Emergency Department. <br />When a small spill is identified the following process will be followed: <br />Staff that discovers the spill will follow S.I.N.: <br />S = Safety First! Clear the area of all non-essential personal. <br />I = Isolate the area by closing doors / windows. <br />N= Notify your Manager / Supervisor and alert the departmental spill response team for clean-up <br />Radiation: <br />No radiology services at this site. <br />Fire / Explosions: <br />Procedures for fire are to be followed as indicated in the Life Safety Policy EC.LS.021-Fire <br />Response Plan-Medical Offices. <br />Emergency Equipment: <br />Facilities must be equipped with the following equipment: <br />An alarm system or internal communications system for providing immediate emergency <br />instruction to emergency personnel. <br />A device for summoning emergency assistance (such as a telephone or two-way radio) <br />immediately available at the scene of operations. <br />Stockton Tommydon MOB Page 4 of 5
