The following is an itemized list of CP22 - HW LQG violations that have not been addressed for FLAG CITY TRUCK SERVICE as of August 07, 2025. Open violations from June 19, 2025 inspection Violation#5-3020002-Facility personnel are trained annually and maintain required documentation until closure. OBSERVATION: The business operator could not produce documentation during the inspection that demonstrates employees have complete hazardous waste training as specified in this section. Required training documents were also not provided during the inspection. REGULATION GUIDANCE: New employees shall be trained within 180 days of hire and annually thereafter as specified in Title 22 California Code of Regulations section 66265.16.The owner or operator shall maintain training records on current personnel until closure of the facility and training records on former employees for at least three years from the date the employee last worked at the facility. CORRECTIVE ACTION: Immediately train all employees who work with hazardous waste in compliance within the time frames and training materials required by this section. Provide training documentation to the San Joaquin County Environmental Health Department(EHD)within 30 days.The documentation must include a list of training topics, date of training,job title, name of employee,written job description (qualifications and duties), and employee signed (or certified)record that documents the training. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#9-3010010-Signed copy of Uniform Hazardous Waste Manifest kept for 3 years. OBSERVATION: The business operator could not provide hazardous waste disposal records for the past three years during the inspection. The following Uniform Hazardous Waste Manifests were missing: -Manifest Number 030062818WAS with shipment date 12/20/2022 -Manifest Number 030065341 WAS with shipment date 7/18/2023 -Manifest Number 030066931 WAS with shipment date 11/7/2022 -Manifest Number 030070475WAS with shipment date 8/23/2023 REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility which received the waste. This signed copy shall be retained as a record for at least three years from the date the waste was accepted by the initial transporter. CORRECTIVE ACTION: Provide a copy of disposal records for the missing manifests listed above. Provide these records and a corrective action statement to the EHD within 30 days. Page 1 of 8
