BELIEVES THE CONDITIONS POSE A HEALTH AND SAFETY CONCERN <br /> AND REQUESTS IMMEDIATE CLEANING AND PROPER MAINTENANCE. NAME ANONYMOUS BUT LEFTA NUMBER TO <br /> CALL FOR ANY FURTHER QUESTIONS. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> COMMENTS AND OBSERVATIONS <br /> Complaint investigation.Arrived on site and met with Executive Director and Maintenance Director. Discussed reason of site visit. <br /> Trash shoots are located in separate adjacent building away from pool building and restaurant. Observed trash shoot located in <br /> residential section of facility. Trash is from residential source and not affiliated with restaurant or pool. No pests observed in trash <br /> shoot location or walking throughout facility. Complaint to be abated.
