The following is an itemized list of CP22 - AST Non -Qualified violations that have not been addressed for SPEEDCO # 944 as of February 09, 2026. Open violations from June 10, 2025 inspection ❑ This violation was corrected ❑ Supporting documents included This violation will be corrected by (date): Describe actions taken or will be taken to correct violation: Violation #32 - 4020002 -Spill prevention briefings are conducted annually. OBSERVATION: Failure to conduct spill prevention briefing for oil -handling personnel at least once a year to assure adequate understanding of the SPCC Plan, including: 1. Known discharges or failures. 2. Malfunctioning components. 3. Any recently developed precautionary measures. REGULATION GUIDANCE: (f)(3) Schedule and conduct discharge prevention briefings for your oil -handling personnel at least once a year to assure adequate understanding of the SPCC Plan for that facility. Such briefings must highlight and describe known discharges as described in § 112.1(b) or failures, malfunctioning components, and any recently developed precautionary measures. CORRECTIVE ACTION: Discharge prevention briefings for oil handling personnel must be scheduled and conducted at least once a year to assure adequate understanding of the SPCC Plan for that facility. Conduct all required spill prevention briefings. Provide compliance verification to the EHD. ❑ This violation was corrected ❑ Supporting documents included ❑ This violation will be corrected by (date): Describe actions taken or will be taken to correct violation: Violation #57 - 4010021 -Written Inspection/testing procedures kept, inspection/testing records signed, kept for 3 years. OBSERVATION: The facility failed to keep written procedures and records of inspections and tests, including integrity tests, for at least three years. Copies of inspection and testing records for 2024 and 2025 were not found on site. REGULATION GUIDANCE: 112.7(e), Conduct inspections and tests required by this part in accordance with written procedures that you or the certifying engineer develop for the facility. You must keep these written procedures and a record of the inspections and tests, signed by the appropriate supervisor or inspector, with the SPCC Plan for a period of three years. Records of inspections and tests kept under usual and customary business practices will suffice for purposes of this paragraph. 112.8(c)(6) Test or inspect each aboveground container for integrity on a regular schedule and whenever you make material repairs. CORRECTIVE ACTION: Inspections and tests must be conducted in accordance with the written procedures developed in the Spill Prevention, Control, and Countermeasure (SPCC) Plan. Records of these inspections and tests must be signed by the appropriate supervisor or inspector and kept on-site with the SPCC Plan for three years. Immediately locate a copy of all inspection and testing records for the last three years, maintain them on-site, and submit copies to the EHD. Page 3 of 5
