Provide a liner for the waste container in the procedure room. <br /> Notes: <br /> 1. Inspection report is emailed to owner/practitioner at thechameleon method(abg mail.com <br /> 2. Waste Hauler agreement provided after the consultation. Sharps Assure is the sharps waste hauler for the facility. <br /> 3. Bloodborne pathogen certificate was available on site but not posted. Noted to operator to post certificate in a visible <br /> area prior to final inspection. <br /> 4. Operator noted black panel will be install on the wall with the door. Black panel made of black plastic. Ridges in the <br /> black panel are big enough to clean easily. Panel is smooth. <br /> 5. Updated copy of IPCP was provided on site. <br /> 6. No disinfectants were available for review on site. Noted to owner/practitioner that disinfectants listed in IPCP must be <br /> available on site for review during the final inspection. <br /> 7. Final inspection will be scheduled with owner/practitioner once payment for facility permit and practitioner permit have <br /> been received. <br /> 8. Operator is to remove plants that are on the shelf in front of the mirror. No live plants in body art facilities as they can <br /> attract insects.
