Report is emailed to thechameleon method(a)g mail,com <br /> 2. All needles on site had the associated sterilization certificates. <br /> 3. Snacks and drinks were removed from the procedure area. No food, drink or tobacco was observed within the facility. <br /> 4. Procedure sink was supplied with a touchless paper towel dispenser, hot water, and an automatic touchless soap <br /> dispenser. Procedure sink was also permanently plumbed. <br /> 5. IPCP was reviewed on site. <br /> 6. Waste Hauler agreement reviewed on site. <br /> 7. Waste containers are all lined. <br /> 8. Sharps container was observed on site. <br /> 9. All client consent forms contain all of the elements necessary. <br /> 10. BBP was posted within the facility. BBP posted in a frame to the left of the procedure sink. <br /> 11. No absorbable materials were observed on site. <br /> 12. Noted to operator to keep track of the lot numbers and ID's on the client consent forms. Operator noted an extra page <br /> that will be attached to the client consent forms that will include the lot numbers, and she will be taking photos of IDs and <br /> attaching photo copies to the clients consent forms. <br /> Facility may now open. <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
