Pacific Coast Producers - Lod Ammonia Refrigeration
Anhydrous Ammonia Refrigeration System 1/5/2022
No. F C R Consequence Safeguards and Control Measures Suggested Safeguards and
Control Measures
Management Response / Action Taken Estimated
Completion
Date
Assigned To Actual
Completion
Date
4.21 2 2 C Loss of oil cooling
associated with the
compressors.
SC-4 has LIC for oil cooling as a backup.
Ammonia detection system inside engine room.
SC-4 high temperature cutout.
Consider evaluating flow switch
to indicate loss of water for oil
cooling.
Consider evaluating
reciprocating compressor high
discharge and oil temperature
cutout functions.
Have a quote and have issued a PO for this.
Part to be received by October 15, 2024.
Installation by January 1, 2025
Information from SMI log indicates all site flow
indicators were replaced and an additional 2
installed October 7, 2022.
10/15/2024
Install 1/1/25
SMI Flow switch
completed
12/20/2024
6.01 2 2 C Ammonia release at
system pressure, if
vessels are not
isolated.
Procedures for oil draining have been
developed by refrigeration contractor.
Deadman valves are installed at oil draining
locations or a portable deadman valve is used.
Oil draining is an attended procedure.
Consider acquiring a portable
eyewash/shower station,
drench hose or an additional
fixed installation unit.
A portable eyewash station is installed in the
compressor room.
Jul-22 T. Tran Jul-22
6.03 2 2 C Compromise Personnel
safety
Two exits from cold rooms. Egress issues not
expected. Oil draining conducted prior to
season before bins are in the room.
Consider the need for a buddy
or attendant in addition to the
technician for performing oil
draining.
SOP #111 states this is a one person job. SOP
#111 has been updated to state this is a two
person job.
10/31/2024 SMI 10/8/2024
7.02 2 3 B If diffusion tank
pressure rating is
exceeded and fails,
potential for ammonia
vapor release at system
head pressure. If tank
doesn't fail and system
pressure builds,
another component in
the system may fail if
component pressure
ratings are exceeded.
PRVs set pressure are equal to or less than
vessel MAWP.
Manual emergency blowdown switch.
Emergency fire control box with equalization
valve.
See Node for recommendation
4.15.
Pressure relief valve calculations provided and
show that the valve design is adequate.
10/10/2024 SMI 10/10/2024
7.04 2 4 B Personnel safety,
Inadequate notification
to facilitate immediate
emergency response.
PM program includes testing ammonia
equipment alarms. Semi annual calibration of
ammonia detectors and alarms. Email and text
call out for engine room ammonia detectors.
Consider upgrading the
ammonia detection systems to
add in audio/visual alarms
outside cold storage rooms,
notification on PLC and alarms
of a PRV release at the
diffusion tank.
On 2/15/2024 SMI completed horns/strobes for
cold rooms. These are inside the room. There
is audio notification.
By 10/15/24 SMI will upgrade ammonia
detection to add notification on PLC of diffusion
tank.
1/1/2025 SMI 2/15/24
Horns/strobes
PLC for
diffusion tank
completed
10/25/2024
8.02 2 3 B Personnel safety Ventilation fan installed in engine room. Consider upgrading the
ventilation fan to and updraft
fan that meets the ammonia
detection activation setpoints
and continuous and emergency
ventilation flow requirements of
IIAR 2.
Fan in place meets requirements at the time of
installation. Fan has already been converted to
an "ON-AUTO" switch.
SMI is replacing the fan and motor with
nonsparking components. Parts have a 14-
week lead time, so this task will be completed
by March 2025
Mar-25 SMI Mar-25
8.05 1 1 D Potential damage to
ammonia system.
Facility is located within 500 year flood zone
with an anticipated depth of less than one foot.
Consider developing or
updating the Flood Emergency
Response Plan to include
shutdown and/or protection of
the ammonia refrigeration
system.
The ammonia system is only in operation from
June - September each year. There is no
possibility that a flood event will occur during
the time the system is in operation; therefore,
the system is in shutdown mode when
possibility for flood event to occur. The
recommendation is not necessary to protect the
health and safety of the employer's own
employees, or the employees of contractors.
Declined
8.10 1 3 C Potential damage to
ammonia system.
Design basis of system met seismic restraint
requirements at time of construction. Added
seismic restraints to piping. Seismic
assessment conducted and repairs verified by
engineer.
Design basis complies with local building code
requirements.
Consider reaching out to CUPA
on when the next seismic
assessment revalidation is due.
The last seismic assessment
follow up was completed in
2019 after the 2016 seismic
assessment.
Seismic assessment completed in 2022,
recommendations completed and engineer re-
evaluation completed 9/18/2024. Engineer
recommended additional repair of exposed
rebar. Photos to engineer on 9/26/24; we will
receive the report after photos are provided.
Received confirmation report from engineer on
10/10/2024 that recommendations were
completed.
9/18/2024
Report received
10/10/2024
8.16 2 3 B Personnel safety and
inability to isolate
equipment/valves
resulting in an
increased release
potential
Emergency stop switch is located outside the
engine room.
