Haza Saeed is an inspector for San Joaquin <br />County Environmental Health Department and cannot be listed as the Environmental Contact. <br />- According to General Manager Jonathan Franco, James Amistoso is no longer employed by the facility. James <br />Amistoso is currently listed as the primary environmental contact. <br />CORRECTIVE ACTION: Complete and electronically submit the Business Activities Page and/or Business Owner <br />Operator Identification Page. <br />This is a (Minor) Violation. <br />10 HSC 6.95 25506, 25505(a)(1), 25508(a)(1), 25508(a)(3) 1010004-Complete and accurate Hazardous Materials Inventory <br />information electronically submitted <br />OBSERVATION: The business failed to electronically submit complete and accurate chemical inventory information for <br />all hazardous materials on site at or above reportable quantities. <br />- In the Hazardous Materials Inventory on CERS the CO2 said to be located in the Kitchen, this is inaccurate. General <br />Manager Jonathan Franco stated that the CO2 is located in the beer cooler room. <br />CORRECTIVE ACTION: Electronically submit complete and accurate chemical inventory information for all hazardous <br />materials on site at or above reportable quantities. <br />This is a Repeat (Class 2) Violation. <br />13 HSC 6.95 25505(a)(2), 25508(a)(1), 25508(a)(3) 1010005-Site Map with all required content electronically submitted <br />OBSERVATION: The business failed to electronically submit a site map with all required content including: north <br />orientation, loading area, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency <br />shut offs, evacuation staging area, hazardous materials/waste storage areas and emergency response equipment. <br /> CORRECTIVE ACTION: Electronically submit a site map with all required content. <br />This is a Repeat (Class 2) Violation. <br />14 19 CCR 1 5030.9; HSC 6.95 25505(a)(3), 25508(a)(1), 25508(a)(3) 1010010-Electronically submitted adequate response <br />plan/procedures for release/threatened release of hazmat <br />OBSERVATION: The business failed to establish and electronically submit adequate emergency response procedures <br />for a release or threatened release of a hazardous material. <br />- Sections C2, C6, C7, C8 are not complete <br />- Section K is signed by James Amistoso, an individual no longer employed by the facility. <br />CORRECTIVE ACTION: Establish and electronically submit adequate emergency response procedures for a release or <br />threatened release of a hazardous material within 30 days. <br />This is a Repeat (Class 2) Violation. <br />1868 E.
