Underground Storage Tank Program Inspection Report <br />Date: <br />July 24, 2025 <br />Facility Address: <br />601 E KETTLEMAN LN, LODIKETTLEMAN CHEVRON <br />Facility Name: <br />Environmental Health DepartmentAMENDED <br />1/22/2026 <br />11 2637(a-d), 2712(b)(1)(F) 2030048 - Sec containment testing conducted per requirements & records kept for 36 mths <br />(TCR 9d) <br />OBSERVATION: Secondary containment testing was completed on 1/29/2024 and 6/11/2024, the test results were not <br />found on site at the time of inspection. <br />REGULATION GUIDANCE: A copy of the test report must be kept on site for 36 months from the date that the testing <br />was performed. <br />CORRECTIVE ACTION: Immediately locate the test results and ensure it is kept on site for 36 months from the date that <br />the testing was performed. Provide a corrective action statement to EHD. <br />This is a Repeat (Minor) Violation. <br />14 2637.2(d&e), 2665(a&b) 2010018 - Overfill prevention equipment inspection report form submitted to the UPA within 30 <br />days <br />OBSERVATION: A repair overfill prevention equipment inspection was performed on 6/11/2024 and a test report was <br />submitted on 8/7/2024, 27 days late. <br />REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. <br />CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in <br />a timely manner. <br />This is a (Minor) Violation. <br />25 2631(g), 2632(c)(2) 2030016 - Annular space of USTs are continuously monitored with an audible and visual alarm <br />system (TCR 9d) <br />OBSERVATION: the 407 annular sensor for the 91 tank was in alarm after being tested and failed to turn off. <br />REGULATION GUIDANCE: The interstitial space shall be monitored with a monitoring system connected to an audible <br />and visual alarm system. <br />CORRECTIVE ACTION: Service Technician Steve Miller replaced the 407 sensor with a 409 sensor at the time of <br />inspection. No further action needed. <br />This was corrected on site. <br />This is a (Minor) Violation. <br />30 2630(d), 2641(a) 2060015 - Leak detection equipment located so leak is detected at the earliest possible opportunity <br />(TCR 9d) <br />OBSERVATION: The 208 sensor located in the 87 STP sump was raised approximately 6 inches off the lowest point of <br />the sump and not located to detect a leak at the earliest opportunity. <br />REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br />opportunity. <br />CORRECTIVE ACTION: The sensor was relocated to the lowest point of the sump by the service technician Steve Miller. <br /> Ensure that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity.
