Underground Storage Tank Program Inspection Report <br />Date: <br />July 24, 2025 <br />Facility Address: <br />601 E KETTLEMAN LN, LODIKETTLEMAN CHEVRON <br />Facility Name: <br />Environmental Health DepartmentAMENDED <br />1/22/2026 <br />59 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly maintained (TCR 9d) <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site, the following test <br />results were missing: <br />- Monitoring System Certification Test Results from 7/29/2024 and 8/28/2023 <br />- Secondary Containment Test Results from 6/11/2024 and 1/29/2024 <br />- Overfill Prevention Test Results from 6/11/2024 and 1/26/2024 <br />- Spill Container Test results from 7/29/2024, 8/28/2023 and 6/5/2024 <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br />(3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings <br />of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings <br />and reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br />three years on site. Submit copies of any missing work orders and alarm records to the EHD. <br />This is a Repeat (Class 2) Violation. <br />69 2712(i) 2030033 - A current UST monitoring plan has been submitted and is readily accessible at the facility <br />OBSERVATION: An approved copy of the monitoring plans were not available for inspection. <br />REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times. <br />CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br />This is a Repeat (Class 2) Violation. <br />72 2712 2030039 - Compliance with all operating permit requirements <br />OBSERVATION: The facility is not in compliance with the written conditions of the operating permit. <br />- The written conditions of the operating permit explains that written records of all monitoring performed shall be <br />maintained on site by the operator and be available for inspection for a period of 36 months from the date that the <br />monitoring was performed, not all records were available on site at time of inspection. <br />REGULATION GUIDANCE: The owner and operator of underground storage tank (UST) systems shall comply with all <br />requirements of the permit issued for the operation of the UST system. <br />CORRECTIVE ACTION: Immediately ensure all written records of monitoring performed remains available on site for a <br />period of 36 months from the date that the monitoring was performed. Provide proof to EHD <br />This is a (Class 2) Violation. <br />77 2712(f) 2030073 - Facility has corrected cited violations within 30 days of receiving report <br />OBSERVATION: An inspection was last done on 7/29/2024 and an inspection report was issued identifying information <br />to be submitted to bring this site into compliance. This information was required to be submitted by 8/28/2024. This <br />information has not been received resulting in a non-compliant status for this facility. <br />REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to submit a written <br />response that includes a statement documenting corrective actions taken or proposing corrective actions which will be <br />taken. <br />CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is submitted <br />within 30 days of receiving an inspection report. <br />This is a Repeat (Class 2) Violation. <br />1868 E.
