CITY OF LATHROP <br /> Above Ground Completions <br /> 1. The Contractor shall excavate a 5-foot square hole extending 3 inches below <br /> the ground surface. A 5-foot square wood form 6 inches deep shall be placed <br /> over the excavation to form the concrete pedestal above the ground surface as <br /> shown in the Plans. <br /> 2. A protective cover constructed of a steel pipe (riser) with a minimum wall <br /> thickness of 3/16 inches and a minimum of 4-1/2 feet long and a locking lid shall <br /> be installed over the monitoring well as shown in the plans. The minimum <br /> diameter of the riser shall be 14-inches for a triple completion monitoring well. <br /> 3.The riser shall be suspended above the top of the well such that the top of the <br /> riser is 2 feet above grade. A concrete slurry shall then be poured and tamped on <br /> top of the annular seal such that the riser is set in concrete. The concrete shall <br /> be poured and tamped until it is approximately 3 inches below grade. The <br /> remaining portion of the excavation and the wood form shall then be poured with <br /> concrete slurry, tamped and finished with a graded surface which shall slope <br /> gently away from the riser. <br /> 4.The finished concrete pedestal shall be 9 inches in thickness at 6 inches above <br /> grade. The riser set in the center of the pedestal shall extend 2 feet above grade. <br /> The steel riser shall be painted with a high-grade rust resistant paint of a neutral <br /> color approved by the Engineer. <br /> Flush Mount Surface Completions <br /> 1. At grade (flush mount) well completions will be housed in a traffic rated valve <br /> box with a cast iron lid and locking ring. The valve box will be a Morrison Series <br /> 519 manhole or approved equal. <br /> 2. The Contractor shall excavate a hole large enough to allow for a 6-inch apron <br /> of concrete around the manhole at ground surface to a depth of 16-inches to <br /> allow for the installation of the specified manhole. <br /> 3.The box will be centered over the well casing and set flush with existing grade. <br /> 4. A concrete slurry shall then be poured and tamped on top of the annular seal <br /> and brought to grade level such that the box is set in concrete. <br /> LUHDORFF&SCALMANINI <br /> CONSULTING ENGINEERS 17 <br />
