The following is an itemized list of violations that have not been addressed for SHELL <br /> FOOD MART as of April 28, 2026. <br /> Open violations from February 24, 2026 inspection <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and current Certification of Financial <br /> Responsibility or other mechanism of financial assurance. During the inspection, Manager Kelli Flesher stated that the <br /> facility has changed owners, effective 10/1/25.The Certificate of Financial Responsibility document and Letter from the <br /> CFO have not been updated with the new owner information. <br /> REGULATION GUIDANCE : The owner or operator must submit a current Certification of Financial Responsibility and <br /> maintain all applicable supporting mechanisms available for review by the Unified Program Agency or the Board upon <br /> request, or as otherwise required by the federal act. <br /> CORRECTIVE ACTION: Submit a complete and current Certification of Financial Responsibility, or other mechanism <br /> of financial assurance within 30 days. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> OBSERVATION: The following float&chain failed to stop the flow of product at the dispenser when tested. <br /> -Diesel in dispenser 6/8 <br /> -87 in dispenser 1/3 <br /> -87 in dispenser 2/4 <br /> REGULATION GUIDANCE: Mechanical release detection equipment used to continuously monitor under-dispenser <br /> containment which fails to function properly at any time, cannot be repaired and must be replaced with continuous <br /> electronic release detection equipment. <br /> CORRECTIVE ACTION: Continuously monitor interstitial spaces of USTs and piping as required and submit verification <br /> of compliance. Fuel should not be dispensed from any dispenser that is not continuously monitored. <br /> Page 2 of 3 <br />
