Assess Analyze Act <br /> 'JOB STEPS zPOTENTIAL HAZARDS 3CRITICAL ACTIONS <br /> 1 b. Position largest vehicle to protect against oncoming <br /> traffic. <br /> 1 b. Face traffic, maintain eye contact with oncoming <br /> vehicles, use a spotter, and establish a safe exit route. <br /> 1 b. Observe potential overhead and ground surface features <br /> that may interfere with moving equipment. Clear the path <br /> of physical hazards prior to initiating mobilization. <br /> 1c. CAUGHT: 1c. Make sure driver has engaged parking brake and placed <br /> Personal injury from pinch wheel chocks in a position to prevent movement. Remove any loose jewelry. Avoid wearing loose clothing <br /> and/or ensure loose clothing is secure. <br /> 1c. Secure all items on the equipment,tighten up any items <br /> or features that have potential to shift or break during <br /> mobilization. <br /> 1d. OVEREXERTION: 1d. Use body positioning and lifting techniques that avoid <br /> Muscle strains while muscle strain; keep back straight, lift with legs, turn with <br /> lifting/carrying equipment. whole body, keep load close to body, and never reach <br /> with a load. <br /> 1d. Watch for heat stress symptoms(muscle cramping, <br /> Weather related injuries. exhaustion, dizziness, nausea, rapid and shallow <br /> breathing). Take breaks in cool places and hydrate as <br /> needed. <br /> 1f. Watch for cold stress symptoms (severe shivering, <br /> slowing of body movement,weakness, stumbling or <br /> inability to walk, collapse). Take breaks in warm areas <br /> as needed. <br /> 1f. Wear clothing appropriate for weather and temperature <br /> conditions(e.g., rain jackets, snow pants, multiple <br /> layers). <br /> 1f. If lightning is observed,wait 30 minutes in a sheltered <br /> 1g. EXPOSURE: location (car is acceptable) before resuming work. <br /> Personal injury from noise 1g. Wear hearing protection if sound levels exceed 85 dBA <br /> hazards. (if you must raise your voice for normal conversation). <br /> Each Job or Operation consists of a set of tasks/steps. Be sure to list all the steps needed to perform job. <br /> A hazard is a potential danger. Break hazards into six types: Contact-victim is struck by or strikes an object; <br /> Caught-victim is caught on,caught in or caught between objects;Fall-victim falls to ground or lower level(includes slips and trips);Exertion-excessive strain or stress/ergonomics/lifting <br /> techniques;Exposure-inhalation/skin hazards;Energy Source—electricity,pressure,compression/tension. <br /> ' Using the first two columns as a guide,decide what actions or procedures are necessary to eliminate or minimize the risk. List the recommended safe operating procedures. Say exactly what needs to <br /> be done-such as"use two persons to lift'.
