Larger equipment with an increased <br /> operating or tip-over radius may need a larger exclusion zone. This should be defined prior to operating each piece of equipment <br /> Assess Analyze Act <br /> 'Ann&W& 2POTENTIAL HAZARDS 3r'01 TICAL ACTIONS Ed <br /> 1. Prepare for machine activity. 1a. CONTACT: la. Cordon off the work area with safety barrels/cones and a rigid <br /> Obstructions in the work area may barrier(snow fence,traffic bar,etc.). Communicate that only <br /> create contact hazards from necessary personnel should be in the work area.Spotter and <br /> machinery. equipment operator shall enforce the 10-ft(exclusion zone) <br /> EZ. Operator will not operate but shall remain in the hands-off <br /> mode while personnel are within the exclusion zone. <br /> 1 b. Fall <br /> Slip/Trip/Fall 1 b. Ensure that work area is flat, level and clear of any obstructions <br /> or debris before setting up work zone. <br /> 2. Spotting. 2a. CONTACT: 2a. Discuss the specifics of the work with the operator and be clear <br /> Machine or load contact with about any hand signals that will be used.Clearly discuss the <br /> personnel, property,or machinery. limits of the assigned work area and the machine's Exclusion <br /> Zone. Maintain Exclusion Zone. The Exclusion Zone shall be <br /> delineated by using 42-inch traffic cones/barrels and a fixed <br /> rigid barrier. <br /> 2a. The Minimum Heavy Equipment Exclusion zone is 1Oft.if it is a <br /> larger piece of equipment or has an increased swing or tip-over <br /> radius the exclusion zone will need to be increased to <br /> accommodate the full range of motion. <br /> 2a. Both the spotter and equipment operators shall have 2-way <br /> radios/cellular devices on their persons to ensure audible <br /> communication in the event any changes or new hazards may <br /> arise. <br /> 2a. All workers should stay outside of the Exclusion Zone of all <br /> equipment unless operator is stopped and in"Hands Off'mode. <br /> (This includes the spotter unless an exception has been <br /> established in the Site-specific JSA). If the Exclusion Zone <br /> must be reduced due to work area restrictions then the spotter <br /> and operator shall enforce the reduced Exclusion Zone. <br /> 2a. Spotters must make eye contact with the machine operator or all <br /> movement ceases until visual contact can be reestablished. <br /> 2a. Spotter shall keep an eye out for any issues with the machine <br /> the operator may not see and communicate with other work <br /> crews and spotters on behalf of the operator. <br /> 2a. If the spotter needs to take a break,he must find a replacement <br /> before leaving or have the machine stop operations. No heavy <br /> equipment shall operate without a spotter under any <br /> circumstances. <br /> 2a. Wear fluorescent clothing/safety vest. <br /> ' Each Job or Operation consists of a set of tasks/steps. Be sure to list all the steps needed to perform job. <br /> z A hazard is a potential danger. Break hazards into six types: Contact-victim is struck by or strikes an object; <br /> Caught-victim is caught on,caught in or caught between objects;Fall-victim falls to ground or lower level(includes slips and trips);Exertion-excessive strain or stress/ergonomics/lifting <br /> techniques;Exposure-inhalation/skin hazards;Energy Source—electricity,pressure,compression/tension. <br /> ' Using the first two columns as a guide,decide what actions or procedures are necessary to eliminate or minimize the risk. List the recommended safe operating procedures. Say exactly what <br /> needs to be done-such as"use two persons to lift".
