Pre-Clearance Protocol. la. CONTACT: 1a. Confirm that(if applicable)"Call Before You Dig"and <br /> Damage to underground utility; confirm positive responses from local utility companies <br /> property damage; personal injury prior to subsurface work in order to confirm utility mark <br /> outs. Must have a case#before digging. <br /> 1a. Review pre-clearing checklist form and sub-surface <br /> clearance form. Pre-clearing protocol indicates that <br /> clearance must be conducted to a minimum of 5 vertical <br /> 1 b. FALL: feet below ground using hand tools. <br /> Slip, Trip or Fall may cause 1 b. Be aware of the conditions when walking or loading <br /> muscle strains or tears, abrasions, equipment and working. Walk within established pathway <br /> lacerations, or broken bones. avoiding uneven surfaces. Remove potential slip/trip/fall <br /> hazards. <br /> 2. Mobilize/demobilize and 2a. The Roux Project Manager will then <br /> recommend additional precautions. <br /> 3b. Wet concrete while using saw to minimize dust and wear <br /> dust mask to prevent inhalation. <br /> 3b. Stand upwind and keep body behind saw. Observers <br /> and helpers should avoid line of fire for saw blade. <br /> Always cut away from body. <br /> 3b. No open flames/heat sources. <br /> ' Each Job or Operation consists of a set of tasks/steps. Be sure to list all the steps needed to perform job. <br /> ' A hazard is a potential danger. Break hazards into five types: Contact-victim is struck by or strikes an object; <br /> Caught-victim is caught on,caught in or caught between objects;Fall-victim falls to ground or lower level(includes slips and trips);Exertion-excessive strain or stress/ergonomics/lifting <br /> techniques;Exposure-inhalation/skin hazards,energy source;Energy Source—electricity,pressure,compression/tension. <br /> Using the first two columns as a guide,decide what actions or procedures are necessary to eliminate or minimize the risk. List the recommended safe operating procedures. Say exactly what <br /> needs to be done-such as"use two persons to lift". Avoid general statements such as,"be careful'. <br /> � ROUX � <br />
