A=COM <br /> • Motors are turned off, unless required for performing maintenance or repair. <br /> • All ground-engaging tools are grounded or securely blocked. <br /> • Controls are set in a neutral position and brakes are set. <br /> • Electrically driven equipment is installed with provision for tagging and locking out the controls <br /> while under repair. <br /> • Manufacturer's requirements for maintenance and repair are followed. <br /> • If continued operation is essential to the process, a safe means of protection shall be provided. <br /> • Provide and use a safety tire rack, cage, or equivalent protection when inflating, mounting, or <br /> dismounting tires installed on split rims, or rims equipped with locking rings or similar devices. <br /> 4.8.5 All heavy equipment shall have a documented inspection and if necessary, repaired prior to use. <br /> • Operators shall not operate heavy equipment that has not been cleared for use. <br /> • All machinery and mechanized equipment will be verified to be in safe operating condition <br /> (refer to S3AM-309-FM1 Approval of Machinery&Mechanized Equipment)by a competent <br /> person (refer to S3AM-202-PR1 Competent Person Designation)within seven days prior to <br /> operation on a new site or project. Clearance is valid for up to one year for the given site or <br /> project. <br /> • As applicable, all machinery and mechanized equipment shall be inspected/certified and <br /> tested at appropriate intervals as required by the manufacturer and/or regulatory requirements. <br /> 4.8.6 All heavy equipment shall be inspected at a minimum to the manufacturer's recommendations prior <br /> to each work shift.All defects shall be reported to the Supervisor/Manager immediately. <br /> • Defective heavy equipment shall be immediately tagged and taken out of service until repaired. <br /> • Inspection, maintenance, service and repair records shall be maintained at the site. If a <br /> manufacturer's or company-specific inspection checklist is not provided, use S3AM-309-FM2 <br /> Heavy Machinery Pre-Operation Checklist. <br /> • Records shall be made available for review upon request. Note: Documents may be <br /> electronically stored in the project files. <br /> 4.9 Fueling and batteries <br /> 4.9.1 A well-ventilated area shall be used for refueling. <br /> 4.9.2 Only the type and quality of fuel recommended by the engine manufacturer shall be used. <br /> 4.9.3 Fuel tanks shall not be filled while the engine is running.All electrical switches shall be turned off. <br /> 4.9.4 If there is potential to spill fuel on hot surfaces, the surfaces shall be permitted to cool down prior to <br /> fueling.Any spillage shall be cleaned before starting engine. <br /> 4.9.5 Spilled fuel shall be cleaned with cotton rags or cloths and disposed of in the proper receptacle; do <br /> not use wool or metallic cloth. <br /> 4.9.6 Open flames, lighted smoking materials, sparking equipment or any other type of ignition source <br /> shall remain a minimum of 35' (10.7m)from the fueling area and/or fuel source.This clearance <br /> shall be increased if required or conditions warrant. <br /> 4.9.7 Heaters in carrier cabs shall be turned off when refueling the carrier or the drill rig. <br /> 4.9.8 Portable containers to be filled shall be placed directly on the ground or be properly grounded prior <br /> to filling to prevent creation of a static charge.
