A=COM <br /> drilling operations. Other indicators of subsurface installations and utilities are the presence of <br /> warning tape, pea gravel, sand, non-indigenous material, bentonite, red concrete(indicative of <br /> electrical duct banks)and any departure from native soil or backfill. <br /> 4.13.2 If unanticipated concrete/debris is encountered and/or if a void is encountered, drilling will be <br /> immediately discontinued and the Manager notified. Drilling may only proceed with Manager or <br /> SH&E Manager approval. <br /> 4.14 Use of Manual Slide Hammer <br /> 4.14.1 The following health and safety procedures should be followed when using a manual slide hammer <br /> to install shallow injection points, drive point piezometers, and drill tools: <br /> • Only use a manual slide hammer that either attaches directly to the point/piezometer being <br /> driven or that incorporates a cap on the point/piezometer/drill tool that prevents the slide <br /> hammer from slipping off the point/piezometer/drill tool. <br /> • Always grasp the manual slide hammer(handles if equipped with handles)with both hands <br /> while driving the point/piezometer/drill tool. <br /> • Never allow hands or feet to get between the manual slide hammer and the drive plate or anvil. <br /> 4.15 Use of Augers <br /> 4.15.1 The following general health and safety procedures should be followed when supervising borings <br /> with continuous flight hollow-stem augers: <br /> • Never place hands or fingers under the bottom of an auger section when it is being hoisted <br /> over the top of the auger section in the ground or other hard surfaces such as the drill rig <br /> platform. <br /> • Never allow feet to get under the auger section that is being hoisted. <br /> • When augers are rotating, stay clear of the rotating auger and other rotating components of the <br /> drill rig. Never reach behind or around a rotating auger for any reason. <br /> • Use a long-handled shovel to move auger cuttings away from a rotating auger. Never use <br /> hands or feet to move cuttings away from a rotating auger. <br /> • Do not attempt to remove earth from rotating augers.Augers should be cleaned only when the <br /> drill rig is in neutral and the augers are stopped from rotating. <br /> • Loud noises may occur while driving split spoons. At minimum hearing protection shall be worn <br /> when driving split spoons. <br /> • When pulling/lifting augers, a clevis pin or other closed device shall be used. Use of J-hooks <br /> is prohibited. <br /> 4.16 Attaching and Breaking Rods <br /> 4.16.1 Do not use manual tools(e.g., pipe wrenches)in combination with rotation of the drill stem. Manual <br /> tools are not designed for the load, and may break. <br /> • The use of such tools creates a significant impact hazard for those in the work area, because <br /> they rotate with the drill stem. Manual tool use in combination with a rotating drill stem to <br /> attach or break rods is therefore prohibited. <br /> • Manual tools may be used if the drill stem is isolated/positively disengaged. <br /> • Mechanical means of rod separation that are permitted include: <br /> o Opposing hydraulic controls. <br /> o Rod locking devices or machine's power vice. <br /> o Hydraulic breakout tools. <br /> o Hydraulic foot clamps. <br /> Drilling,Boring,&Direct Push Probing(S3AM-321-PR1) <br /> Revision 2 July 31,2019 <br /> PRINTED COPIES ARE UNCONTROLLED.
